Thanks for renewing your Walk with a Doc program! Please complete the information below to renew your chapter.
Step 1: Complete the Renewal Form

Fill out the form below or click here to open it in a new window. Make sure you read and complete all 4 pages of the form. Once you complete the final page (the Walk Census) a blue “click to sign” button that looks like this will appear.

Note: The standard renewal paperwork is set up to renew your chapter for one year. If you’re interested in signing a multi-year agreement, please email contact@walkwithadoc.org.
Step 2: Renewal Payment
Choose a membership option below. The annual renewal fee is $250. You also have the option to add a merchandise kit to support your local walks. We never want cost to be a barrier. If you’re unable to pay the full renewal fee this year, please continue your program and consider contributing what you can – a donation of any amount is appreciated and helps us support more communities.
